Health Dialog Personal Health Assessment

Health Dialog was looking to redesign and reposition and modernize their Personal Health Assessment (PHA) product to better position it in the marketplace. The team worked to transform the traditional PHA survey into a product that benefited the end user by allowing them to participate in the creation of a health plan. The simple three point plan offers the user health information along with relevant online and offline employer/insurance wellness programs.
